a. The image should be 3.5cm x 4.5cm in size.
b. The photo must have plain light background (white/light blue or light grey background)

|
|

b. Child’s Birth Certificate number |
|
|

c. OBA Category - OBA membership number (printed on the card) |
|
|
|
d. Brothers Category
- Admission numbers of the siblings studying in the school (Zahira College) at
present |
|
|
|
e. Staff Category
- Employee number |
|
|
|
f. A Valid Email address |
|
|
|
g. Photographs of parents and the child |
|
|
|
h. Credit/Debit card for online application fee payment ( Rs. 2,500/- ) non refundable |
|
|
|
|
|
|
|
04. Completing application form |
|
|
|
a. You cannot proceed to the next section of the form unless all mandatory fields (as marked by *) in the current
page are completed |
|
|
|
b. Once above information are available click ( APPLY NOW ) to
proceed with the application form and then click SUBMIT to send the
application online.
(Print immediately after submission or email the completed application to your personal email to print on later) |

c. DO NOT use the browser back/forward buttons to navigate an Application that is in the process of being completed. |

b. Pre School Certificates (If attended) |

c. Progress & Educational Report of the Brothers/ Sisters of the
child |
|
|
|
d. Copy of Birth Certificates of Brothers/ Sisters |
|
|
|
e. Latest Facility Fees Voucher of the Brothers who are studying in
the school |
|
|
|
f. Parents’ Marriage Certificate |
|
|
|
g. Educational and Extracurricular Certificates of the Parents |
|
|
|
h. Employment and Remuneration proof of parents |
|
|
|
i. If Business (B.R. Certificate/ Company Registration Certificate/
Tax Receipt |

|

j. If Father is an Old Boy, Old Boy’s ID Card or Payment Receipt |

k. House Title Deeds/ Tenancy agreement |

l. Electricity Bill/ Water Bill |
